The form includes basic listener information by default, but the fields on the form are customizable.

If your station chooses to collect a signature for a W-9 when sending the form to a listener, the form will automatically include and require information that appears on the W-9: name, address, date of birth, and Social Security Number. (The form's validation will reject fake SSNs like 000-00-0000.)

We recommend that you include the station logo at the top of the page.

To add fields to the form, click and drag the field titles from the list of Available Fields on the left to the Included Fields on the right. Once in the list of Included Fields you can drag titles up or down to re-order, and click into the checkbox to the left of the field title to make that field required.

To remove a field, click the red 'X' icon to the left of the field title.

You also have the option to include a form Footer where you can provide additional instruction to your listeners.

Finally, you can change the "Thank you message" which a listener sees after submitting info through the form.

When you're satisfied with the changes scroll to the top to save.

If you would like to apply these changes to the other stations to which you have access you can use the Copy to Sister Stations icon.
